These insurance plans typically cover all services (if authorization is obtained) that Horizon offers, but there are occasionally carve-outs and limitations based on the specifics of every unique plan.

Do I need pre-authorization for residential treatment?

Yes, prior or pre-authorization is required for all services.  Some plans may not authorize services for Horizon due to network contract services HMO vs PPO plans, and other limitations payers can place on individual policies.

Can I use out-of-network facilities with Aetna insurance?

Yes. We access OON (out-of-network) plans if we can get prior authorization.  

What happens if my plan doesn’t fully cover the treatment?

This is handled on a case-by-case basis. Most often Horizon is willing to accept what the plan pays as reimbursement. Each case is unique and every potential client is evaluated and the maximum financial liability the client may face is outlined and written down in a good faith estimate prior to admission.
